How do you write a Military Memorandum for Conditional Release to Active Duty?

My husband is in the Army National Guard and is trying to go active duty Army. His DD 368 was working it's way up the COC with signatures but at the last signature(Brigade Commander) the BC couldn't sign it because the recruiter signed his spot. So now, SFC of my husband's unit told him he can still get all the signatures as long as he gets another 368 form. My husband has now turned it in as of yesterday to the SFC but now SFC wants a memorandum about why he wants to go active to go along with the DD 368 because another soldier in the unit has requested to go active too but wrote a 4 page memorandum! Of course, the husband has never written one and we want to make a good impression. So how do we write one? Any formal headings?
